Meet Nicole Martin MSN, APRN, FNP-C
Nicole began her healthcare journey as a pharmacy technician, gaining early expertise in medication safety and accuracy. She later built a strong foundation in hands-on patient care as a nursing assistant, where her commitment to compassion and clinical excellence first emerged.
Over the years, Nicole advanced from Registered Nurse to Family Nurse Practitioner, dedicating her career to home health and the geriatric population. She has held progressive clinical leadership roles—including Case Manager, Clinical Supervisor, Assistant Director of Nursing, and Director of Nursing in home health—where she consistently enhanced patient outcomes, care coordination, and operational quality.
Her experience also includes work as a Telehealth Nurse Assessor and Director of Clinical Operations, expanding her impact through technology-supported care and innovative service delivery.
Through extensive primary-care home visits, Nicole identified a critical and growing need for specialized diabetes support among seniors and homebound patients. This insight led her to expand her scope into diabetes specialty care, offering comprehensive management, education, and complication prevention directly in patients’ homes. She is passionate about empowering patients to understand their condition, improve daily management, and reduce avoidable hospitalizations.
Today, as a dedicated Nurse Practitioner and Primary Care Provider, Nicole delivers personalized, relationship-centered care. She tailors every treatment plan to the patient’s unique needs and environment—prioritizing safety, comfort, independence, and long-term wellness through a holistic and compassionate approach.

Meet Britney Ebanks MSN, APRN, FNP-C
Britney is a compassionate Family Nurse Practitioner with broad clinical experience across outpatient, inpatient, and community health settings. She is skilled in assessing, diagnosing, and managing acute and chronic conditions for diverse patient populations, with a strong foundation in cardiovascular care, symptom management, and patient education. Her approach emphasizes evidence-based practice, health promotion, and holistic wellness.
Her background includes work as an NP in a specialty outpatient clinic and several years of nursing experience in hospice, home health, critical care step-down, and telemetry. She has provided comprehensive assessments, coordinated transitions of care, delivered skilled nursing interventions, and collaborated closely with interdisciplinary teams to improve patient outcomes.
Britney holds a Master of Science in Nursing from South University. She remains dedicated to patient-centered care, clear communication, and supporting patients and families across the continuum of care.
