CCM & RPM Programs
- Chronic Care Management (CCM): Monthly Medicare-covered program for chronic conditions, care coordination, medication review, symptom monitoring, reduces hospitalizations, $0 cost to patient.
- Remote Patient Monitoring (RPM): At-home devices track glucose, BP, weight; NP reviews daily, early alerts for abnormal readings; ideal for diabetic patients with fluctuating sugars; fully covered by Medicare.
